The Health Care Home Navigator is a healthcare professional who practices within the scope of the Health Care Home Navigator role. She/he must follow any licensure of certification rules. She/he is able to develop and foster positive working relationships with patients and other care team members. She/he must possess the ability to care for and give direction to and coordinate care for assigned patient and teams. She/he is competent to provide care to patient in the age ranges to which she/he is assigned.
Qualifications:
Graduate of an accredited school of registered nursing Current Minnesota Registered Nurse License Greater than five years of nursing experience preferred, preferably in ambulatory and hospital care ACLS certified preferred Ability to work both independently and collaboratively as an effective member of a health care team Excellent verbal, written, and communication skills Strong interpersonal skills required Strong organizational and prioritization skills required Ability to provide care for all age levels and functional abilities
Job Responsibilities:
Utilizes skills of observation, interview, patient history, and physical assessments/observations to collect data from the patient and/or others to identify variables affecting care. Conducts and documents data in patient charts. Forwards information to appropriate team members for proper follow-up. Assesses the educational needs of the patient (i.e., learning readiness) and provides patient and/or significant others with information about their condition and/or care as evidenced in the patient record. Demonstrates flexibility to adapt to change. Exhibits qualities of dependability. Comments constructively in peer evaluations. Maintains an organized work environment by being accountable for delegation and completion of tasks. Stays current with the requirements of Health Care Home rules/regulations. Assists with training of new staff and ongoing training of staff regarding Health Care Home’s patient/family-centered care. Performs medical documentation and phone triage per OMC policies. Maintains confidentiality. Coordinates and works cohesively with the care team to create and update individualized patient Care Plans. Works cohesively with other Health Care Home. Provides approved education information to the patient and other care team members based on medical conditions, and accurately documents information in the patient chart. Is an active care team member that contributes to team cohesiveness and follows proper chain of command.
