The Clinical Nurse Specialist (CNS) follows the ANA Standards of Nursing Practice for Registered Nurses and Advanced Practice Re! gistered Nurses. In addition to these functions, and depending on the administrative and clinical needs of the organization, the CNS functions as clinician, educator, consultant, researcher, and leader.

HCMC's Adult Partial Hospital Program is a time-limited, structured program of psychotherapy and other therapeutic services specifically designed to meet the mental health needs of persons in an acute crisis. The program provides short-term (approximately 3 weeks) intensive mental health treatment as an alternative to inpatient hospitalization or as an option following inpatient hospitalization.

Clinical

Collaborates with members of the health care team to coordinate the care of patients with complex disease processes or issues across the continuum of care.

Conducts a holistic assessment that integrates knowledge of health and illness with appropriate therapeutic interventions.

Participates in clinic visits and rounds on inpatient! s in accordance with the care management guidelines for the sp! ecific patient.

Serves as a clinical nursing expert and resource to staff.

Communicates with referring health care providers regarding comprehensive plan of care for patient and family.

Works closely with physicians and departments to deliver coordinated care in a timely manner.

Works with multidisciplinary team to develop quality monitors to measure patients outcomes and program performance.

Develops networks for referring patients and families in the community.

May prescribe, administer and dispense therapeutic devices and drugs in accordance with the CNS/Physician collaborative agreement.

Participates in performance improvement activities.

Education

Consults with and teaches patients and families about their diagnosis, disease process and plan of care. Collaborates with psychiatrist, social worker and occupational therpist regarding specific teaching needs.

Participates in ongoing e! ducational activities to expand advanced clinical knowledge and enhance role performance.

Assesses ongoing staff competence and collaborates on development of educational activities to improve care

Serves as a resource for the community to provide public education.

Utilizes current research, scientific literature and advanced practice knowledge when providing education for patients, families, community and staff.

Consultation

Assists patients to navigate through the program and services/procedures needed during diagnostic workup and course of treatment.

Provides phone consultation, advocates for care or referrals to patients, families, other health care professionals and the community.

Serves as Program Liaison to community through a variety of communication sources.

Provides consultation regarding patient care to clinical and/or patient care areas.

Leads interdisciplinary groups in designing a! nd implementing innovative alternative solutions that address system pr! oblems and/or patient care issues occurring across the full continuum of care.

Research

Participates in clinical research for program as required.

Collaborates with colleagues and other health care team members in identifying, developing, implementing and utilizing research findings.

Disseminates relevant research findings through practice, education and consultation.

Leadership

Collaborates with the multi-disciplinary team to develop, implement and evaluate resources for program patients.

Facilitates communication and collaboration with multiple disciplines to enhance program growth and development.

In collaboration with multi-disciplinary team, participates in ongoing program development.

Establishes a mechanism to assure ongoing communication regarding the patients progress with all providers of the patients care, assuring the transition of care between settings.

Monitors outcomes a! nd resource utilization for individual or groups of patients in measuring quality and cost effectiveness of care provided.
