ABOUT THIS OPPORTUNITY

In this position, you will provide new and continuing training leadership to the Claims Services area.

The Care Coordinator Supervisor Trainer is an important part of the quality process for the claims department, assisting in performing quality reviews.

In addition, the Care Coordinator Supervisor Trainer manages the assessment process and coordinates follow-up on referred clients. The Care Coordinator Supervisor Trainer is the main contact person for the client and develops and manages the plan of care, assists clients with finding services and follows the client telephonically at regular intervals a! s directed.

RESPONSIBILITIES

Training and! quality responsibilities:
Provides input and assistance to develop and maintain all orientation manuals.

Conducts orientation with all new Care Coordinator Supervisors.

Contributes input and assistance regarding cross training and retraining for all Care Coordinator Supervisors.

Provides the ongoing training of issues such as productivity, quality, etc.

Plans CEU and quality meetings, prepares materials, and delivers information at meetings and via e-mail.

Participates in the quality check process for new and existing employees.

Participates in User Acceptance Testing for IT projects as directed.

Other training duties that are assigned.

Responsibilities when not training

:

Maintains quality (95% with a goal of 98%) and productivity (50%) department standards on a consistent basis.

Reviews onsite assessments for consistency and quality and develops plan of care in coll! aboration with the field nurse.

Identifies and assesses clients’ health care needs across a continuum of care.

Prepares a professional, objective report for the insurance customer summarizing information from the assessment using company formats. The report is based on the basic principles of case management and the Five Steps of the nursing process: assessment, diagnosis, planning, implementation and evaluation. Understands how the medical conditions, functional and cognitive deficits affect an individual’s safety and how to incorporate this information into an individualized, comprehensive plan of care.

Makes the initial contact with the client/family/contact to set the expectations for the assessment process and evaluate need for immediate provider services.

Coordinates and implements home care services and community resources for clients across a continuum of care.

Provides ongoing regular evaluation of appropriateness of! services as they relate to changing health conditions of clients. Upda! tes the individualized plan of care on an ongoing basis to reflect changing client care needs.

Obtains information from physicians, family members, third-party payers, caregivers, or other health care providers (such as social worker, adult daycare worker, Medicare nurse) as needed to prepare a comprehensive plan of care and ongoing updates related to care coordination.

Partnership Requirements

An Access Agency shall ensure the selection of qualified staff.

a.

The care manager who conducts the assessments, develops care plans, and provides ongoing monitoring shall be either a registered nurse licensed in the State where care management services are provided or a social services worker who is a graduate of an accredited four-year college or university.

The nurse or social services worker shall have a minimum of two years of experience in health care or human services.
A bachelor’s degree in nursing, health, social work, gerontology or a related field may be substituted for one year of experience.

b.

Care managers shall have the following additional qualification:

i.

demonstrated interviewing skills which include the professional judgment to probe as necessary

to uncover underlying concerns of the applicant;

ii.

demonstrated ability to establish and maintain empathic relationships;

i

ii.

experience in conducting social and health assessments;

iv.

knowledge of human behavior, family/caregiver dynamics, human development and disability;

v.

awareness of community resources and sevices;

vi.

the ability to understand and apply complex service reimbursement issues; and

vii.

the ability to evaluate, negotiate and plan for the costs of care options.

c.

Care manageme! nt supervisors shall meet all the qualifications of a care manager plus! have demonstrated supervisory ability, and at least one year of specific experience in conducting assessments, developing care plans, and monitoring home and community based services.

Maintains appropriate organization confidentiality.

Required to uphold the principles of compliance as outlined in the Code of Conduct, Employee Handbook and related policies and procedures.

Supports and participates in the mandatory Corporate Compliance Program training initiative on an annual or more frequent basis, as required.

Performs work accurately and efficiently under deadline pressures.
Experience

:

·

Associate degree or diploma in Nursing or Bachelors Degree in Social Work

·

Experience:
o

Registered Nurse: 2 years of nursing experience required

o

Licensed Soci! al Work: 5 years of geriatric social work experience

o
Master Licensed Social Worker: 2 years of geriatric social work experience

·

Current, valid and unrestricted Registered Nurse or Social Worker license in the State of Minnesota.

·

3 years of previous Care Coordinator Supervisor experience is required.

·

Must type at least 40 words per minute.

Preferred Experience:
·

Bachelor or Master’s Degree in Nursing or Master’s Degree in Social Work.

·

2 years of previous leadership and/or training experience is required.

·

Prior experience in home health care, geriatrics, assessments, knowledge of community resources, experience in various clinical areas, case management, insurance, and mental health

·

Advanced practice or specialty certification in the areas of gerontology, case management, rehabilitation, community health, insurance, home health, and hospice and palliative c! are.
