The Physical Therapist evaluates, plans, treats and implements care for patients in accordance with prof! essional standards of hte AAPT using Clinical Practice Guidelines, MN Boardof Medical Practice and policies and procedures of Essentia Health. Settings include: Home Health, Nursing Home, Acute Care, Outpatient Clinic, Schools and other contracted facilities.
Provides Physical Therapy services to patients, ages infant through adult, including evaluation and assessment, treatment, education, and consultation to improve clients’ abilities to their highest feasible level. The therapist provides services as part of a care team assembled to assure optimal customer service, and clinical and financial outcomes, under the direction of SMDC service line manÂagement and the referring physician. Position requires high level of customer service skills to establish and enhance positive relationships with patients, co-workers and others. JOB QUALIFICATIONS/EDUCATION/EXPERIENCE REQUIREMENTS:
Education/Experience Bachelor’s Degree/entry-level Master’s Degree in Physical Ther! apy from an APTA-approved program.
Licensure/Crede! ntials: Current Minnesota Physical Therapy license. Current certification in BLS certified or become certified within 6 months.
