Home Infusion Resource Center Coordinator

Description: Fairview Health Services is at the momment seeking for Home Infusion Resource Center Coordinator right now, this vacancy will be placed in Minnesota. More details about this vacancy opportunity please read the description below. Coordinate the delivery of nursing services for all FHI patients using an interdisciplinary team approach. Responsibilities include discharge planning, educa! ting patients and caregivers, and serving as a resource to referral sources, regional agencies, and patients. Clinical resource for all nursing issues related to patients on service with home infusion.
Job Qualifications:
Current RN licensure in state of Minnesota. BLS (Basic Life Support) Certification.
Minimum of two years recent nursing experience in an acute care setting, perferably in critical care, oncology, or outpatient infusion. Prefer infusion experience in an acute care setting or ambulatory care setting.
