Sanford Health of Northern Minnesota, Family Medicine Department is seeking a RN Health Coach to Provide guidance, education and expertise in regards to healthcare choices to imp! rove the quality of patients' lives. this role provides counseling and developed action plans to reach patient goals for healthier living. Utilizes nursing knowledge and excellent communication skills to coordinate between health care providers and the patient ensuring patient satisfaction and maximum care. Applies clinical experience and judgment to Health Coach process.Requirements: Must have graduated from an accredited school or college of nursing.
Bachelor's degree in nursing is preferred. Specific services/positions may have additional education, training or experience requirements. Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum or have had a minimum of 400 hours of experience as a RN (or LPN) at another facility. Must be licensed as an RN in Minnesota.
If not certified as a Health Coach at time of hire, incumbent must attend Health Coach Training and obta! in certification within 12 months of hire.
