Sholom, one of the leading senior housing and healthcare service organizations in the Twin Cities, is seeking a full time RN Grant Project Coordina! tor to join our team at our beautiful 17-acre St Louis Park campus.
DUTIES
The RN Grant Project Coordinator assures that the Resident Centered Care Connections Program (RCCCP) is successfully implemented within the nursing facility, completes requirements as set forth by the RCCCP Work Plan such as implementation of Interventions to Reduce Acute Care Transfers (INTERACT II), Physicians Orders for Life-Sustaining Treatments (POLST), Palliative Care Program, and Spiritual and Ethical care.
Primary Responsibilities
Establish and implement processes to reduce unnecessary readmissions to the hospital by improving the identification, evaluation, and communication about changes in resident status.
Enable appropriate and effective transitions between care settings.
Improve the structure of palliative care services and advance care planning.
Collect and submit data as determined by the Resident Centered Care Connections Program (RCCCP) task! force.
Attend and participate in Resident Centered Care C! onnections Program (RCCCP) meetings and task forces.
Maintain involvement with Steering and Advisory Committees as directed.
QUALIFICATIONS
Education, Experience and Credentials
Bachelor's degree in Nursing or Health Care Administration, or related field required.
Three or more years experience in Nursing and/or Health Care experience required.
Must have and maintain current licensure in the State of Minnesota as an RN.
Certified Hospice and Palliative Nurse (CHPN) Certification or working towards.
CPR/First Aid certified through the American Red Cross preferred.
