River Falls Area Hospital (RFAH) is located in River Falls, Wis., just 30 minutes east of St. Paul, Minnesota. RFAH has 20 staffed beds, 210 employees and more than 156 affiliated physicians. The shared medic! al campus includes primary care and specialty clinics, a long-! term care facility and a wellness and fitness center. A broad range of inpatient and outpatient hospital services are provided to approximately 28,000 patients each year.
Responsibilities
Coordinate the patient's journey through the acute care continuum
Works with multiple disciplines, departments, and external agencies to meet the needs of the patient and family
Establish patient care relationship with all patients needing care management coordination
Qualifications
Education / Experience:
Current WI RN license
Graduate of accredited nursing program
3 years of recent direct patient care experience in RN role
Prior experience in care management or care coordination preferred
BLS certification required
Medical Terminology required
Skills:
Demonstrated strong customer service, team work, and interpersonal skills
Strong verbal and written c! ommunication skills
Able to work independently
Pay,Benefits, & Work Schedule
