Location: Minneapolis Minnesota

Description: Clinicor is presently looking of Triage - Registered Nurse right now, this vacancy will be placed in Minnesota. Detailed specification about this vacancy opportunity kindly read the description below. Bilingual Hmong or Spanish Triage - Registered Nurse will work under the direction of the nursing supervisor.
The Triage RN will handle phone calls for patients who are seeking medical i! nformation and/or medical attention. This individual will schedule appointments as the patient's condition requires, answer questions and provide limited health education patients with medical questions.

Clinicor is seeking an individual with several years of pediatric experience and chronic disease knowledge, preferably, someone who has completed telephone triage in a clinical setting who will be able to sensitively complete telephone encounters.•Provide telephone triage by assessing signs and symptoms of illness and directing patients to an appropriate plan of care.

  • Work with multiple providers, staff, and collaborative partners in a culturally diverse environment to develop and maintain a plan of care to diverse and under-served patients; deliver skilled independent and interdependent professional nursing care.
  • Assess patient/family/care giver/community knowledge and factors affecting the ability to learn health related issues; devel! op and implement a teaching plan.
  • Assess discharge ne! eds; develop and implement a discharge and follow-up plan.
  • Document data, plans, patient progress, response to care, and other relevant patient information.
  • Communicate and carry out patient care orders.
  • Evaluate response to planned care and teaching and collaborate with the health care team to adjust plans as needed.
  • Make or request referrals as appropriate.
  • Delegate aspects of nursing care to appropriate member of the nursing staff.
  • Supervise and evaluate the delivery of delegated care; work as charge nurse as assigned; participate in quality improvement activities.
  • Graduated from an accredited school of nursing.
  • A current State of Minnesota nursing license .
  • At least three years of experience as a registered nurse in a health care setting, preferably primary care, and working with culturally diverse, under-served, and at-risk populations.
  • Strong interpersonal and customer service sk! ills.
  • Excellent assessment skills; ability to recognize signs, symptoms, and prioritize patient for care.
  • Bilingual skills in English and Hmong and/or English and Spanish preferred.
  • Experience:
?As a registered nurse providing phone triage in a clinical setting.

?Working in a pediatric care setting.

?Using the Epic electronic health record system.

  • Ability to:
?Be cross trained to work in other units.

?Work at a computer for long periods of time.

?Use professional judgment to delegate tasks appropriately.

Clinic Hours:
Monday through Friday 8am to 7:30pm and Saturday morning between 8am to 12pm.
