MDS Coordinator - RN
Location: Saint Paul Minnesota
Description: Benedictine Health System is currently seeking to employ MDS Coordinator - RN right now, this career will be placed in Minnesota. Responsible for the overall coordination of each resident's MDS/RAI care plan scheduling, to assist with implementation, completion and transmission. Responsible for case mix review! in determining reimbursement case mix levels. Effectively interacts with residents, family members and other health care team members while maintaining standards of professional nursing. 48 hours a 2 week pay period - Monday - Fridays
Qualifications:
Graduation from an accredited school of professional nursing. Current RN license to practice nursing from the Minnesota Board of Nursing, which must be in good standing. Experience in geriatric nursing CPR certified Knowledge of current state and federal regulations governing long-term care facilities Ability to utilize MS Word, MS Excel, MS Outlook and Achieve Matrix Knowledge of MDS, Medicare and care-planning process. Ability to communicate effectively with residents, families, and interdisciplinary team Must be able to read, write and communicate in English Must be able to maintain confidentiality (HIPAA)
Skills and Abilities:
Previous MDS experience extreemly helpful
