The Hennepin County Health Care for the Homeless of the Human Services and Public Health Department is seeking a Family Nurse Practitioner who has an interest in, and skill and experience working with, underserved populations. This position will provide health care services at local shelters and drop-in centers serving homeless clients in the downtown Minneapolis area. Preventive health and acute care services are provided on a walk-in basis to adult men and women, youth, children and infants. The Health Care for the Homeless Nurse Practitioner is the highest credentialed provider on site working with a team of Registered Nurses, Licensed Practical Nurses and Medical Assistants.

This is a benefits earning, limited duration (temporary) position with a duration of up to two years employment. Guaranteed hours are flexible, anywhere from 20 to 40 hours per week. Work hours are Monday through Friday and include working two or three ! evening clinics per week. No weekend, overnight or holiday shi! fts are required. The successful candidate must be able to travel among shelters and drop-in centers to provide medical services.

This list may be used to fill future full-time, part-time, permanent, and limited duration positions.

The primary duties and responsibilities of this position include: Perform initial and ongoing assessments of patient medical, physical and psychosocial status. Triage patients who require physician supervision. Obtain and evaluate patient health and medical history and document in the EPIC Electronic Health Record (EHR). Perform physical examinations and conduct preventive screening procedures based on age and medical history; identify ! medical and health risks. Complete accurate and timely documentation of observations, interventions, referral and follow-up; develop patient care plan. Order appropriate therapeutic treatments; order, conduct and interpret diagnostic tests. Prescribe, administer and dispense therapeutic devices and legend drugs. Provide health education, counseling, consultation and advocacy based on needs identified in care plan. Refer clients to other health professionals and community agencies. Assist with ongoing medical and nursing staff teaching programs; initiate and participate in research studies. Develop and maintain positive relationships with community service providers. Serve as an active team member of a multi-disciplinary team by attending meetings; actively participating in program planning, development and evaluation; and assisting with reports and special projects. Best Qualified Candidates will have: Master's degree or higher in nursing from an accredited school of nursin! g AND at least 24 months of full-time professional nursing experience w! ithin the area of specialization. Possession of a valid Minnesota license as a Registered Nurse. Current certification as a Family Nurse Practitioner. Current CPR certification. Sensitivity to the needs and issues of diverse and disenfranchised individuals. Strong leadership, verbal and written communication skills. Ability to work independently, counsel and instruct individuals in a state of stress, and act as an advocate. Ability to establish and maintain effective relationships with internal and external partners.
