The clinical Staff Telepharmacist performs daily pharmacist functions using standard techniques and procedures to process writte! n prescriptions issued by physicians, dentists, and other qualified prescribers. Provides information, keeps pertinent records, oversees medication preparation and generally protects the well being of patients by checking for adverse reactions, allergies, overdoses, and inappropriate use and dose of prescribed medications. Uses clinical skills to monitor medications and dose selected by order of the physician or approved medical staff protocols

.

Responsibilities:
·

Responsible for patient safety concerning drug overdose, adverse reactions, allergies, and appropriateness of drug use and drug interactions.

·

Responsible for all equipment used in the COE including computer equipment, calculators, and telepharmacy equipment.

·

Reviews written prescriptions in order to determine appropriate ingredients needed and to assure that overdoses have not been prescribed or that toxic compounds will not result ! from combining ingredients.

·

Demonstrates ! the competency to provide care appropriate to all age populations of the patients served. Age

·

populations may include: Neonatal, Pediatrics, Adolescence, Adult and/or Gariatrics.

·

Maintains, when possible, and monitors patient drug profiles using computerized programs in order to assure patient safety.

·

Uses pharmacokinetic skills to clinically monitor patients and doses of selected drugs under order of the physician or by approved medical staff protocols in order to assure safe and consistent use of toxic drugs.

·

Gives direction to technicians, students or residents in day to day work related activities, using professional judgments as to priorities of work flow in order to increase productivity and foster a team concept.

·

Attends mandatory workshops and meetings sponsored by the Corporation and also continued education in pharmacy practice in order to meet regulator! y standards and COE/hospitals goals.

·

Supports and assists in meeting COE goals for order entry, medication preparation and clinical programs

·

Gives a thorough, concise report at change of shift and communicates incomplete assignments, and potential patient concerns to monitor in order to ensure continuity of medication therapy.

·

Assures that all order types are processed in a timely and accurate manner.

·

Investigates and reports adverse drug events and medication errors identified.

·

Provides appropriate information regarding drug therapy when consulted

·

Reviews and interprets all physician orders received, using the patient profile. Screens all medication orders for optimal usage and intervenes when appropriate within the standards of the clinical programs:

o

Allergies-appropriately documented in computer

o

Drug interac! tions

o

Checks renal and liver function for dosing mo! difications

o

Duplicate orders discontinued.

o

Monitors for incompatibilities, concentration and rate.

·

Participates in the maintenance of continuous quality improvement programs.

·

Performs other duties as deemed necessary by the director of COE or the Fargo Division management in accordance with established client hospital policies and requirements of accrediting bodies.

Qualifications

Education:
·

B.S. in Pharmacy or equivalent required from an accredited College of Pharmacy

·

Must be a registered pharmacist in good standing with the North Dakota State Board of Pharmacy and Minnesota.

Must be willing to obtain Licensing in all states as appropriate. A (90) ninety day training period is desired to familiarize the candidate with technology and communication functions and pharmacy procedures.

·

ASHP Accredited Resid! ency Training preferred

·

A (90) ninety day training period is desired to familiarize the candidate with technology and communication functions and pharmacy procedures. Contributes to work functions during the training period.

·

Clinical skills, knowledge and abilities include understanding the composition and properties of drugs and their pharmacologic actions.

Experience:
·

3 years of Hospital Pharmacy Experience required

·

Experience in Coordinating Patient Care Activities preferred

·

Communicates on a professional level with physicians, nurses, and other ancillary professional staff when discussing drugs and drug related problems.

·

Numerical ability - form perception and clerical ability to follow lengthy procedures when using complicated computer programming.

·

Analytical abilities - be able to interpret written prescriptions ! and be able to concentrate at a high level to prevent possible drug rel! ated errors while entering orders.

·

Must be able to distinguish between proper and improper drug selection and appropriateness of use. Restricted by laws governing the dispensing of pharmaceuticals, but otherwise has freedom to consult with prescriber to resolve problems. Is governed by work load standards by policy and productivity.
