The Home Health Aide is responsible for performing personal care, assistance with advanced daily living skills (ADL's) and/or light housekeeping and to help maintain a healthful, safe enviro! nment for Home Care/Hospice clients of all ages.
Implements and abides by Customer Service Standards.
Supports and implements patient safety and other safety practices as appropriate.
Supports and demonstrates Family Centered Care principles when interacting with patients and their families and with co-workers.
EDUCATION
High school diploma or equivalent preferred.
Successful completion of Certified Nursing Assistant (CNA) or Home Health Aide/Homemaker course preferred.
EXPERIENCE
Three months experience in Home Care setting or equivalent preferred.
SPECIAL REQUIREMENTS
Reliable transportation is required for this position. A valid Minnesota driver’s license, a driving record in good standing, and the minimum amount of insurance liability coverage required.
Must meet Home Health Aide/Homemaker competencies as defined by Medicare.
Current certification as a Certified! Nursing Assistant or Certified Home Health Aide strongly pref! erred.
Current American Heart Association (AHA) BLS Healthcare Provider card preferred.
