Under the direction of the Manager of Admissions and Business Development and the Director of ! Programs and Services, the supervisor provides programmatic and operational leadership and direction to the CKRI SW and CC to ensure patients and clients receive care in the post-acute continuum which meets their personal needs to health and return to the community.
Responsible for meeting regulatory, clinical and contractual objectives for internal and external providers; state and federal agencies; and managed care payers
In tandeum with the Director, manager, physicians and PACs, assists in developing and implementing strategies to standardize processes to achieve outcomes leading to a greater population health, e.g. those with SCI
In partnership with the Director of Business Development assure for the most fiscally responsible method to achieve lifelong care planning, focusing on using resources to remain an active member of the community
Education
Bachelor’s degree in nursing, social work or health care related field
Master’s degre! e required or actively completing graduate studies in a health! related field
Qualifications
Experience
Current RN, Social Work or other clinical licensure
Skills
Minimum 5 years of related healthcare experience:
Demonstrated proficiency in working in matrixed operations
3 years care management experience preferably in an integrated healthcare delivery system
3 years in Rehabilitation preferred
Demonstrated track record in leading and delegating a team with accountability and successful achievement of superior patient care, operations, and financial results
Demonstrated experience of effectively working with physicians
Ability to lead groups from various professional backgrounds, including leadership of teams without formal “line†authority
Experience working with key strategic partners and independent physician groups
Advanced communication skills
Possesses a positive attitude which promotes excellence
Demonstrated ability to influence individuals an! d groups at all levels in the organization without formal organizational authority
Skilled decision making capability that allows for openness, resolution, and ethical decision making, with high personal integrity; together these attributes create, build and foster trust within and across multiple entities
Conflict management and resolution skills
Pay,Benefits, & Work Schedule
