SHIFTS: PT Evenings, Days and PT Overnights, Casual on-call available
- All regularly scheduled shifts require every o! ther weekend and every other holiday.
Follows the mission, policies and procedures and safety rules of the facility. Understands/supports the Resident Bill of Rights Follows Federal and state ofMinnesotarules and regulations governing nursing facilities. Provides resident cares per individualized plan of care. Follows facility policy and procedures regarding: Bathing Grooming Oral hygiene Dressing Ambulation Transfers RangeofMotion Bed Mobility Toileting Feeding
Assists resident to meet nutritional needs: Residents who need help in feeding will be given assistance as stated on the care plan. Fresh water and other fluids will be offered as appropriate. Nourishments/supplements will be passed on a timely basis. Changes in appetite and intake will be reported to the team leader. Accurate I and 0s will be recorded and reported to the team leader. Weights will be accurate and completed on a timely basis. Organizes workload and ! sets priorities for resident to assure workload is completed o! n a timely basis. Uses safety standards: Uses transfer belts for ALL transfer/ambulation of residents. Uses proper body mechanics. Ambulates/transfers resident per plan of care. Positions residents to maintain body alignment per policy and procedure and plan of care. Follows infection control policy/procedures. Follows resident care protocol. Properly cleans and stores resident equipment. Maintains sanitary conditions in common areas. Follows basic principles of linen handling. Follows principles of universal precautions. Reports any concern related to IC to team leader. Establishes and maintains respectful communications with all employees, residents, and families. Attends required inservices. Attends/participates in continuing education opportunities to meet 12-hour minimum Federal requirement per year. Reports resident care information to team leader on a timely basis. Observes and reports changes in resident condition immediately to team leader. Shows respect for reside! nts rights to confidentiality, dignity and privacy. Documents care/information per policy and procedure. Reports to team leader prior to and at end of shift. Uses proper chain of command to communicate concerns. Completes any other duties delegated by nurse. Participates/supports activities of facility committees.
Qualifications
Active and in good standing on the Minnesota Nursing Assistant Registry. Must be at least 18 Years Old Must be able to safely perform the essential functions of the job Satisfactory completion of facility orientation and specialized program orientation
