Responsibilities:
Provides primary health care in collaboration with physician to residents in long term (LTC) and Assisted Living (AL) settings.
Provides clinical management and on site visits in collaboration with physician to patients in transitional care settings (TCU).
Provides timely documentation in electronic record.
