Key Responsibilities include:
- Monitoring resident care staffâs job performance
- Conducting associate performance evaluations, and coaching/counseling associates
- Monitoring staff in administration of medication
- Observing residentsâ health status, taking actions to address concerns, and reporting all significant changes
- Communicating to families regarding residentâs personal needs and achievements. Reminds families of need for scheduled physician, dental, or eye appointments.
- Assisting in organizing and developing service plan reviews
- Observing residents, recording significant reactions, and notifying appropriate parties of residentâs condition and reaction to drugs, treatments, and significant incidents
- Responding promptly to all emergency calls from resid! ents
- Performing daily, weekly and monthly safety and infection! control checks of the home and grounds
- Must have current Practical Nursing Licensure in the state of MN
- Prefer Memory Care LPN experience
- One year of experience
- Must have supervisory skills
- Must be proficient with computers and software to include Excel and Microsoft Office
- Must have excellent communication skills
- Must be organized, possess problem solving skills and be able to multi task
- Must enjoy working with the senior population and have a passion for working with residents, families, and professionals
