- This is a full time Work at Home opportunity for the right candidate.POSITION SUMMARY: With a focus on partnering with our! Care Delivery Design team this role will ensure clinical programs and workflows are delivered accurately and on time. Responsibilities will include executing new workflows and/or processes to meet the ACS business results. This role will foster strong, ongoing client relationships due to the ability to manage high value initiatives that impact the delivery of products and services to the internal and external environment. This position reports into the Accountable Care Solutions (ACS) Care Delivery Lead.
- Responsible for ensuring the ACS Care Delivery programs are implemented to promote the best in class program offerings of our internal partners in order to meet the needs of our external partners (ACOs, provider delivery systems, etc.).
- Collaborate with partners within National Care Management and Active Health Management to: Document the delivery detail necessary to ensure that the ACS programs being provided are c! onsistent with the approved ACS clinical program design; Meet ! the defined requirements; Identify the solutions to overcome any inconsistencies.
- Work collaboratively with other implementation teams including ACS operations, ActiveHealth, and other teams to ensure successful deployment of the ACS Care Delivery Programs (note: both Sr BC and BC).
- Provide guidance and direction to external vendors, external consultants, consulting team members and/or matrixed staff as required in support of ACS Care Delivery initiatives.
- Drive or guide the development of holistic solutions and/or key strategic plans.
- Proactively identify opportunities for improvement and independently influence business leader(s) to act or change behavior through negotiations and/or consultation that promotes issue resolution.
- Collaborate with marketing and sales to create messaging on the value-add of the ACO. This value should be messaged and communicated in a compelling manner so that a favorable response is received. EDUCATION:
- Bachelor's degree or equivalent experience.
- Registered Nurse (RN) is desired.
- Strong Clinical Program experience with a familiarity with clinical program delivery on the provider side.
- Specific experience within an ACO model is highly desired.
- Ability to introduce and implement programs.
- Strong communication skills, both oral and written.
- Strong presentation skills.

- This position requires extensive travel. Travel will be more than 50% of the time.
