SUMMARY: Provides direct care and case management for a team of Agency clients in accordance with the Minnesota Nurse Practice Act, Agency policy, and physician/provider orders.

PRIMARY DUTIES! AND RESPONSIBILITIES:
Reviews client referral information and responds to requests and inquiries as appropriate. Completes comprehensive assessments, including OASIS, within forty-eight (48) hours of referral unless other arrangements are made. Determines appropriateness of referral for home care services and responds per Agency guidelines. Performs initial and on-going client assessments based on Agency policy, standards of practice, and regulatory guidelines to ensure effective and appropriate home care services. Performs comprehensive assessments including OASIS where indicated. Considers the physical, psychological, and socioeconomic needs of the client, level of family acceptance, and ability for client needs to be met in the home. Collaborates with physicians, other health care professionals, clients, and families in developing a comprehensive coordinated plan for care. Identifies and assesses overt and subtle signs of potential physiologic and psychological ! changes. Notifies the physician in accordance with Agency poli! cies and procedures. Makes decisions and/or recommendations reflecting consideration of immediate and long-range effects (i.e., frequency of visits, components of care plan, additional services). Confers with clinical director on a regular basis. Utilizes agency and community resources appropriately. Monitors documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness. Provides on-call support for clients per schedule. Communicates with other disciplines/departments when required. Identifies the educational needs of team mmbers. Provides training or refers to clinical director to arrange specific training program. Performs or delegates in-home supervision of team members. Identifies staffing needs and communicates information. Demonstrates knowledge of safety infection control practices by compliance with policies and procedures. Recognizes and responds appropriately to potentially unsafe situations. Demonstrate! s safe and competent practice in the use of equipment. Assesses safety of environment and takes initiative to prevent accidents and promote safety. Participates in all manadatory education per established guidelines. Maintains confidentiality in all aspects of the job. Does not reveal client information to others who do not have a need to know. Participates in the quality improvement program as directed by Clinical Director/Designee. Follows established Agency guidelines for protection of confidentiality of client information. Directs, plans, and initiates appropriate action responsively in home care situations. Directs/monitors assigned clients clinical team to provide effective and quality home care services. Promotes personal safety and a safe environment for clients and co-workers. Performs job duties in accordance with agency policies and procedures, according to all regulatory guidelines, and within professional standards. Performs other duties as assigned. QUALIFICAT! IONS

Graduate of an accredited school of professional nursing.! Current license, in good standing, to practice as a Registered Nurse in Minnesota. Minimum of one (1) year nursing experience. CPR Certified. Strong communication and interpersonal skills. Licensed driver with automobile insured in accordance with the state and/or Agency requirements and in good repair. Has a driver record acceptable as per Ecumens driver policy.
