Responsibilities :
Provide care management/coordination support to a panel of cancer patients (primarily Prostate) who require specialty services integrated & in support of the overall plan of care through the continuum of the cancer experience. Interacts with & supports the specialty care provider and interdisciplinary care team. Coordination of care & services with the interdisciplinary team to ensure smooth transitions from one service to another within our hospital and clinic systems, provide patient education & enabling informed decision making, provide emotional support & refer patient to appropriate supportive services, enhance flow of information between collaborating physicians, as well as between physicians & patients.
Qualifications :Education/Experience:
Bachelor's Degree in Nursing or other health-related field Minimum 5 years clinical experience required with 2-3 years experience in oncology specialty Previous care management experience preferred BLS required CMS and/or Oncology-related certification preferred Medical Terminology required
