This position in our Specialty Pharmacy operation performs patient care related duties in coordination with pharmacy production. They will perform patient care assessments and provide n! ursing interventions via the phone center within the Specialty Pharmacy.
Responsibilities:
- Receive incoming calls and provide assessments for patients on assigned work list each day
- Reevaluates patients response to therapy as necessary by performing follow up calls with the patient and/or physician
- Enters documentation using configured assessment into the system and summarize as required * Completes patient electronic medical records, including patient assessment documentation and continuing patient progress summaries
- Assess patients in regard to adherence, barriers to using identified therapy, what to expect with treatment and how to use medication correctly including storage requirements
- Answer questions and provide education on various topics including: injection training, common side effect management, and quality of life screenings
- Create, evaluate and revise patient care plans
- Assist th! e LPN's daily on making clinical decisions and indentifying in! terventions in the workflow that require individual care plans to be set in place
- Coordinate with nursing agencies when required
- Triage calls appropriately to Pharmacist or other area when identified
- Participates in the coordination of care to meet patient outcome goals
- Assist in creating standard protocols around common situations
- Other duties as assigned
- Associates degree in Nursing
- Current Registered Nurse License in good standing with the Minnesota Board of Nursing
- 1-3 years experience providing education to patients and/or caregivers
- Bachelor of Science in Nursing
- Prior experience in a specialty drug education or Home infusion setting
- Chronic disease management experience
- Strong familiarity of specialty medications and side effect profiles
- Excellent communicat! ion skills-verbal and written
- Previous phone communication experience
- Long Term Care experience
- Organized, self-motivated and detail oriented
- Strong sense of teamwork
- PC aptitude & Proficient in MS Office Applications
