Allina Health Home Care Services (AHCS) is the trusted provider and expert resource in delivering patient-centered care and services for patients and families needing rehabilitation, advanced-illness or end-of-life care.
With headquarters located in St. Paul, Minn., services are provided in 30 Minnesota counties, 24 hours a day, seven days a week. Responsibilities Home visits to admit patients into Home Care program. RN assessment of patients.
Referral of clients to other disciplines as appropriate. Computer competence / compl! ete and accurate information. All work completed on day of adm! ission. Qualifications Education/Experience: ADN / BSN.
1 year of previous home care experience and / or 3-5 years of nursing experience required. Licensure/Certifications: BLS Driver's License Skills: Customer service knowledge and skills. Knowledge of Medicare, MA and third party payor benefits. Good communication and listening skills.
Ability to apply clinical nursing skills with emphasis on pain and symptom management, IV skills, care of dying patient, general physical assessment skills. Problem solving / critical thinking ability. RN hires with less than 1 year of RN experience must be a graduate of an accredited school of nursing (American Association of Colleges of Nursing or The National League for Nursing) which is inclusive of a clinical rotation in an acute care facility.
