Allina Health Home Care Services provides skilled medical care for patients and families needing rehabilitation, advanced-illness or end-of-life care. With headquarters located in St. Paul, Minn., services are provided in 30 Minnesota counties, 24 hours a day, seven days a week.
Responsibilities
Answer phone calls from patients, families and facilities that are receiving Home Care , Hospice and Senior Care Transitions services from Allina.
- Utilize the nursing process; assessment, planning, intervention and evaluation to coordinate & deliver care to patients in their home or in a facility.
- Practices and demonstrates knowledge of current policies and protocols
Education
- Registered nurse: licensed in MN and eligible for licensure in WI
- BSN preferred
Qualification/Skills
Previous triage telephone experience
- Hospice or home health experience preferred, especially pain & symptom management; oncology experience also helpful
- Excellent customer service skills
- Previous experience using Excellian or Epic medical record
- Ability to communicate effecti vely both written and verbal
Work Schedule
- 28 hours per week
- Day/Evening shift (Day shifts are 7:30-4pm or 8-4:30pm, Evening shifts are 3:30-11:30pm or 4:00-12midnight)
- Every other weekend and 3 holidays annually
- Non-Contract, Exempt
- Benefit eligible
- Work with other triage nurses at St. Paul - Westgate office
